Kurt Muskopf, Clinic Director and staff at the Fairview Heights location are excited to be moving to a new location in Fairview Heights, just a half-mile south from the old clinic. The clinic's new address is:5300 N. Illinois, Suite 101 Fairview Heights, IL 62208 Phone: 618-624-9300 (unchanged) The clinic will be closed on Friday, April 24th and re-open at the new location on Monday, April 27th. |
